Programs for Adults 65 and Over

Alternative Care (AC) Program

The AC Program is a state-funded program that provides home and community-based services for people age 65 years and older who require the level of care provided in a nursing home, choose to live in the community and are not yet financially eligible for Medical Assistance.

Some individuals that qualify for AC program will be required to make a monthly payment called an Alternative Care (AC) monthly fee: A monthly fee required for most people on the AC program to help pay for the cost of services provided under the program. People on AC pay cost-sharing fees that range from 5% to 30% of the average monthly cost of their AC services, unless they are exempt from paying a fee.

Elderly Waiver (EW)

The Elderly Waiver provides funds for home and community-based services for people age 65 and older who require the level of care provided in a nursing home and choose to live in the community. EW promotes community living and independence with services and supports that address each person's individual needs and choices. EW offers services that go beyond what is available through Medical Assistance.

To determine eligibility for a service you are requesting, a MnCHOICES assessment is required. A MnCHOICE assessment needs to be completed by a certified assessor and can be set up by the county you reside in.
