Health and Human Services

A joint effort of Public Health Nurses and Social Workers to administer Pre Admission Screenings and Reassessments of Pine County residents to determine eligibility for Alternative Care (AC), Community Alternatives for Disabled Individuals (CADI), Community Alternative Care (CAC), Traumatic Brain Injury (TBI) and Elderly Waiver (EW), Developmental Disabilities and/or Related Conditions (DD/RC) and to connect residents with services such as habilitation, independent living services, home health services, home delivered meals, respite care, family counseling/training, special adaptations to the home, medically necessary care, services, therapies, and prescriptions, adult day care, assisted living, residential care services, adult foster care, and companion services or nursing home placement.

Community Alternatives for Disabled Individuals

The Community Alternatives for Disabled Individuals (CADI) program is open to eligible applicants under age 65, certified disabled, eligible for MA and require a nursing home level of care; clients may receive medically necessary care, services, therapies, and prescriptions, and special services that are necessary to prevent nursing home placement, like adaptations to the home, extended home health services, and home delivered meals. Expenses can not exceed the cost to MA for nursing home care.

Community Alternative Care

Community Alternative Care (CAC) program is open to eligible applicants under age 65, determined to need a hospital level of care, certified disabled, and eligible for MA; clients may receive medically necessary care, services, therapies, and prescriptions, and special services that are necessary to prevent nursing home placement, like respite care, family counseling/training, special adaptations to the home and extended home health services. CAC is offered to prevent hospitalization when the client prefers community services and shows a cost savings to MA.

Developmental Disabilities and/or Related Conditions

This funding source is a limited resource allocated to the counties by the MN. Dept. of Human Services for administration by the counties for persons who are at risk of nursing home level of care, as an alternative to institutionalized placement.

Respite Care

Is provided to client receiving or enrolled in a specific program.  For eligibility, please contact your case manager.
